Document current state and future state. Work flow. Develop training curriculum. Lead training of team(4 ppl) now that will turn into 20 in the near future.
We are really looking for someone who can help create and document the Care Coordination process/workflow and help create Care Coordinator training guides. Understands Start/Stop/Continue processes.

Qualifications:
Clinical background (preferably nursing or pharmacy)
Excellent physician relation skills required
Leadership experience and/or leadership potential
Self-starter, self-directed & takes initiative
Ability to effectively collaborate with multidisciplinary stakeholders (e.g.: IT, physician offices, physicians, care coordination team)
Skilled in critical thinking & results-oriented process improvement
Excellent oral & written communication skills
Preferred:
Ability to effectively utilize and leverage technology systems & understand data analytics
Familiarity with IT & EMR systems (preferably Allscripts)
Previous experience working in an ambulatory environmentEstimated Salary: $20 to $28 per hour based on qualifications.
